Job summary

Elizabeth Park Rehabilitation and Nursing LLC is seeking a compassionate Licensed Practical Nurse (LPN) for a 7:00 AM to 3:30 PM shift in New Jersey. You will provide direct patient care, administer medications, and help develop care plans in collaboration with the healthcare team.

Ideal candidates hold an active NJ LPN license, with 1+ year in long-term care or rehabilitation, and demonstrate strong communication and empathy for patients and their families.

Qualifications

Current New Jersey LPN license.
1+ year experience in long-term care or rehabilitation.
Ability to lift, transfer, and reposition patients.
Strong communication with patients, families and healthcare teams.

Responsibilities

Provide direct patient care including medications, wound care, and treatments as prescribed.
Develop and implement individualized care plans with patients and families.
Monitor patient progress and suggest care-plan modifications.
Collaborate with RNs, PTs, OTs, and social workers for comprehensive care.
Participate in patient and family education and ensure clear communication.
Maintain accurate records in accordance with standards.
