Job summary

Gateway CSB in Georgia is seeking a licensed Practical Nurse to provide a broad range of paraprofessional nursing duties under clinical supervision. The role includes patient care, intakes, and participation in treatment planning to support consumers and families.

Under RN supervision, you will administer medications, perform nursing assessments, document care, and help with admissions, discharges, and transfers in accordance with facility policies.

Qualifications

• Current LPN license without restrictions.
• 6 months of experience as LPN preferred but not required.

Job Summary

Provides a broad range of paraprofessional social services and nursing duties to consumers and their families. Under the clinical supervision of RN, provides patient care. Conducts consumer intakes, participates in treatment team planning. Assists in dealing with personal and social problems. May perform case management duties. Assist with admissions/discharge or transfer process of client in accordance with policies of facility and unit guidelines/expectations.

Nursing

• Administers medications accurately and safely within agency guidelines.
• Performs treatments and provides preventative maintenance care to consumers per MD orders and accepted Nursing Standard of Practice guidelines.
• Performs Nursing Assessments
• Consistently and accurately transcribes and implements physician's orders and documents all consumer information

Consumer Resource

• Educates consumers regarding disease process, diagnoses, medications and other aspects of care and treatment.
• Uses appropriate interview techniques to obtain pertinent health history

Documentation

• Monitors, records and reports symptoms and changes in consumer's conditions in timely manner.
• Documents pertinent information in record accurately and timely.

Work Environment

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Some minor travel between Gateway sites is required.

Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Employee must frequently lift and/or move up to 15 pounds.
