Job description

Salary: USD $31.25/Hr.

Description and Responsibilities

The Adult Learning Center is a Day Habilitation program serving 45 adults aged 23-85 with diagnoses of intellectual disabilities and some physical challenges. Services include occupational therapy, speech therapy, physical therapy, nursing oversight, functional skill development, communication, socialization, community membership, and self-advocacy. Staff implement structures to maximize participant involvement in activities.

The Licensed Practical Nurse (LPN) will coordinate medical services with each individual's primary care provider or medical clinic, complete nursing assessments and documentation, develop and implement Individual Service Plans, and advise the program director and staff of any medical issues affecting participation.

Other Key Responsibilities

• Administer medications according to physician’s orders.
• Accompany individuals to medical visits as needed.
• Advocate for individuals with physicians and providers.
• Collaborate with RNs to maintain medical records and documentation.
• Provide training on medical issues and procedures, including hygiene and health concerns.

Qualifications

• Licensed Practical Nurse in the Commonwealth of MA, required.
• Physical stamina and ability to lift individuals served.
• Valid Driver's License and acceptable driving record, required.
