Responsibilities:

1. Provide services within the scope of practice as defined by state laws, according to the plan of care, and in coordination with the healthcare team. Do not alter the plan of care.
2. Visit patients as assigned and provide nursing care per the plan of care.
3. Participate in in-service programs as per LHC policy.
4. Complete and submit progress notes daily for each visit.
5. Participate in QA/QI processes.
6. Observe and report patients' nursing needs; instruct families on care and maintaining a healthy environment.
7. Report patient responses to treatment and condition changes to supervising nurse or physician.
8. Adhere to and participate in the Care Management process.
9. Attend staff conferences regarding patient care.
10. Perform all other duties as assigned.

Education and Experience:

License Requirements:

• Current LPN licensure in the state of practice.
• Valid Driver's License, vehicle insurance, and access to reliable transportation.
• CPR Certification.
