The Licensed Practical Nurse (LPN) reports to the Supervising Registered Nurse of the designated location as well as the Clinical Director respectively and shall practice under the supervision of a Registered Nurse (RN) and under the Nursing Practice Act of Washington, DC. This position is primarily responsible for performing nursing activities and person-centered care for the people we support within the agency’s residential facilities. This is a contract position; please only apply if you are interested in being a contractor.Essential Functions:Exercise quality nursing judgment in the application of established policies and procedures in order to ensure well-being and safety of the people that we support.Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing careAdminister medication and treatments accurately and observing and documenting response to interventions.Perform delegated and independent nursing functions using established procedures, policies, guidelines, and professional nursing standards.Contribute to the development of the person’s plan of care by collecting data and conducting focused nursing assessments based on the identified problems and needs.Participate in nursing care management through coordinating preventative care with Primary Care Physicians (PCP), Psychiatrist and other ancillary disciplines.Reconcile medication administration records, Physician order sheets, as well as new Physician orders accurately.Delegate nursing interventions to Trained Medical Employees (TME’s) which includes medication administration and other interventions.Participate in health teachings under the direction of the Registered Nurse to promote, attain, and maintain positive outcomes.Participate and attend staff development trainings and meetings including but not limited to: mock surveys, department meetings, DDS trainings, Human Rights Committee, Psychotropic med reviews, ISP’s and Court appointments.Assist the people that we support and family members/guardians with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.Maintain professionalism through appearance, verbal and written communication.Serve as a LPN preceptor in orienting new nurses joining the organization ensuring that they understand the policies and procedures of the organization.Attendance to mandatory and non-mandatory trainings and in-services (3 annually) to strengthen the knowledge of the community that were serve; maintain the number of required CEU’s for licensure renewal.Organize and maintain medical records according to RCM’s policy.Document appropriate findings as it relates to nursing practice via weekly and monthly progress notes and report vital information to the health care team.Ensure completion and follow up of all consults relating to the person’s health needs.Maintain Medication Administration Records (MAR).Ensure that first aid kits and treatment supplies are in place and ready for use.Participate with quality improvement team to correct any deficiencies.Report all suspected negative allegations to the Clinical Director as well as Incident Management CoordinatorFill in as a Direct Support Professional as needed to assist with person care routines and medical appointments.Serve as an advocate for the people and upholding all “People Rights” in accordance with regulations.Be aware of and safeguard the persons welfare at all times.Participate in all on-going job-related training (all 4 phase of DDS training) and staff meetings with the ability to demonstrate training competencyMaintain current licensure and certifications to effectively perform professional duties (LPN license, Health Certificate, TB testing, CPR and CPI).Other duties as assignedKnowledge Skills and Abilities:Knowledge of Electronic Medical Records System(s)Knowledge of health and wellness standards, Department of Health and Department on Disabilities proceduresExcellent computer skillsBasic mathematical skillsAbility to respond to changing situations in a flexible manner in order to meet current health care needsPossess the ability to employ critical thinking skills (including Crisis Situations)Consider relative costs and benefits of potential actions and choose the most appropriate oneAbility to develop staff by providing constructive feedback and finding opportunities for their growthAbility to effectively present information and respond to questions from various groupsAbility to interpret a variety of instructions furnished in written, oral, diagram, or schedule formAbility to write reports, business correspondences and procedure manualsEffective oral and written communication in the English languageAbility to gain a working knowledge of RCM policies and procedures and act as a representative of RCM in publicExcellent interpersonal skills with the ability to effectively communicate and coordinate with other staff members and people from different backgroundsPossess leadership ability and the willingness to work harmoniously with professional and non-professional personnel and maintain confidentiality; must be organized, have attention to detail and ability to prioritize daily tasksAbility to pass a criminal background checkAbility to lift, move, or assist non-ambulatory people and/or weights with a minimum of 60 lbsWhile performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands for hands on assessment; reach with hands and arms and talk or hear for an undetermined amount of time.Education and Experience:Graduate of an accredited school of NursingCurrent and valid Licensed Practical Nurse license in the District of ColumbiaMinimum of 1 year experience as a Licensed Practical Nurse in a related communityHave a current and maintain annual health certificate stating employee is free and clear of any and all communicable diseasesCurrent American Health Association BLS and CPR certificationAble and willing to complete duties as directed and model professional behaviorMust have patience, tact, cheerful disposition, and enthusiasm, as well as be willing to handle people based on whatever maturity level they are currently functioningPhysical and Environmental Conditions:Work environment may involve situations that require working with people with Behavior Support Plans, Trauma Informed Care Plans, Complex Medical Needs, and Significant Developmental Disabilities.
