Quality Talent Group is partnering with a network of skilled nursing and rehabilitation centers in New York to recruit an experienced Licensed Practical Nurse (LPN). You will deliver direct patient care, administer medications, and help implement personalized care plans in a supportive team.

Qualifications

• Active and unrestricted LPN license in New York or current state of practice.
• Experience delivering nursing care in long-term and skilled nursing settings preferred.
• Familiarity with EHR systems and digital charting required.

Responsibilities

• Provide direct nursing care to residents in skilled nursing and long-term care facilities.
• Administer medications and IVs as prescribed.
• Develop, implement, and evaluate individualized care plans.
• Record vital signs and monitor patient conditions.
• Supervise and mentor other nursing staff when needed.
• Educate residents, families, and caregivers on care plans.

Qualifications:
