Job summary

Quality Talent Group is seeking a Licensed Practical Nurse (LPN) to join a trusted network of skilled nursing and rehabilitation centers in New York. The role includes delivering direct nursing care, administering medications, and implementing care plans in skilled nursing and long-term care settings.

Applicants should hold an active LPN license in the relevant state, with strong documentation, communication, and patient-care skills.

Qualifications

• Active and unrestricted LPN license in the state of practice.
• Experience or capability in medication administration and patient care procedures.
• Strong documentation and communication skills with time management ability.
• Familiarity with EHR and digital charting systems.

Responsibilities

• Deliver direct nursing care to residents in skilled nursing and long-term care.
• Administer medications and IVs as prescribed.
• Implement and evaluate individualized care plans.
• Record vital signs and monitor patient conditions.
• Supervise and guide other nursing personnel when required.
• Educate patients, families, and caregivers as directed.
