Administer medications, including injections and IV infusions, following physician orders and safety protocols.
Conduct comprehensive patient assessments, monitor vital signs, and document medical records accurately using Electronic Health Record (EHR) systems.
Assist with activities of daily living such as tube feeding, catheter care, and mobility support using equipment like Hoyer lifts.
Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident’s needs.
Manage acute pain and coordinate hospice or palliative care services for residents requiring end-of-life support.
Ensure infection control policies are strictly followed, including specimen collection, sterilization procedures, and aseptic techniques.
Communicate effectively with residents, families, and team members to provide updates on health status and care plans.
Valid state licensure as a Licensed Practical Nurse (LPN).
Prior experience working in long-term care facilities such as nursing homes or skilled nursing facilities.
Demonstrated knowledge of medical terminology, anatomy, physiology, and ICD coding standards (ICD-9/10).
Experience with EMR/EHR systems such as PCC for accurate documentation.
Background in dementia care, Alzheimer’s disease management, memory care, and behavioral health is highly desirable.
Familiarity with IV insertion, medication administration, first aid, and basic life support (BLS).
Experience working with geriatric patients or individuals with disabilities is preferred.
