Job summary

togetherwecancareers is seeking an LPN in New York to provide medication administration and nursing care per the established plan. You will work under the Head/Charge Nurse to ensure optimal resident care on the unit.

Your role includes evaluating residents with the nurse manager, documenting all meds and treatments, and adhering to Department of Health regulations. A NY LPN license and >1 year of experience are required.

Qualifications

• Active NY LPN license required.
• Experience with medication administration and nursing care plans is preferred.
• Strong documentation and communication skills with nursing leadership.

Responsibilities

• Administers and documents all medications and treatments per policies and DOH regulations.
• Continuously evaluates residents' behavior, symptoms, reactions and progress with the Nurse Manager/Charge Nurse.
• Performs all treatments per physician orders and established policies.
• Discusses changes or discontinuation of medications with the Nurse Manager/Charge Nurse.
• Orders prescribed meds, notifies pharmacy of discontinuations and ensures accuracy of dosages.

The LPNis responsible foradministration of all medications and treatments (except those specificallydesignatedfor an RN) during their shift, according to the established medical and nursing care plan. Formal Education:Graduated an accredited school of Licensed Practical Nursing
