Livingston County Center for Nursing and Rehabilitation is seeking a Licensed Practical Nurse to provide routine care under a detailed patient care plan.

The LPN works under supervision and administers medications, assists with nursing care plans, monitors vital signs, and supports admissions and discharges, ambulation, and equipment care as part of daily resident care.

Qualifications

  • Graduation from a NYSED-acceptable nursing program.
  • License to practice as Licensed Practical Nurse in NYS.
  • Ability to follow physician orders and medication policies.
  • Ability to document and communicate clearly.

Responsibilities

  • Prepares, administers, and records medications according to the physician’s order and medication policies of the facility.
  • Carries out uncomplicated therapeutic treatments such as giving enemas, preparing and applying simple dressings, and administering certain medications per policy.
  • Assist in or actively implement the nursing care plan for residents/patients.
  • Take temperature, pulse, respiration, and observe and record signs and symptoms as directed.
  • Collect specimens and assist in carrying out diagnostic procedures under supervision.
  • Assist in the admission and discharge of patients.
  • Assist residents/patients in ambulation exercises and maintaining joint motion.
  • Care for equipment used in care of residents/patients as directed.
  • Performs other related duties as assigned.

Job description

RESIDENCY: Candidates must have been legal residents of the following jurisdiction(s) for at least four months immediately preceding the date of the application:

  • Livingston County or New York State

The work involves the routine care of residents/patients in accordance with a detailed and well-defined patient care plan. Assignments concerning resident/patient care are specifically provided with limited leeway allowed for the exercise of independent judgment. Supervision of subordinates will vary with the type of facility providing the resident/patient services. In the Livingston County Center for Nursing and Rehabilitation, the Licensed Practical Nurse works under the direct supervision of a higher-level nursing position. A Licensed Practical Nursed does related work as assigned.

  • Assists in the admission and discharge of patients;
  • Assists residents/patients in performing ambulation exercises and maintaining proper joint motion;
  • Cares for equipment used in care of residents/patients as directed; and
  • Performs other related duties as assigned.
FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES & PERSONAL CHARACTERISTIC

S: Working knowledge of nursing principles and techniques and their relation to medical and surgical practices; working knowledge of hospital sanitation and personal hygiene; ability to carry out therapeutic treatments prescribed; ability to understand and carry out detailed oral and written instructions; ability to communicate effectively both orally and in writing; ability to keep accurate, clear, legible records and make reports; ability to get along well with residents/patients and others; ability to reach, bend, etc.; mental alertness; cheerfulness; good moral character; patience; emotional stability; sympathetic attitude toward the sick; physical condition commensurate with the demands of the position.

Applicants must meet all requirements for one of the following qualification options in order to be approved for examination and/or appointment:

Qualification Option #1
  • Training/Education: Graduation from a nursing education program acceptable to the New York State Education Department (NYSED).
  • General experience: N.A.
  • Supervisory experience: N.A.
  • License/Certification: Licensed by New York State to practice as a Licensed Practical Nurse.
  • Other requirements: See Special Qualifications below.
Qualification Option #2
  • Training/Education: Graduation from a nursing education program registered by the New York State Education Department as licensure qualifying no more than 90 days prior to submission of an application for employment.
  • General experience: N.A.
  • Supervisory experience: N.A.
  • License/Certification: Submission to the New York State Department of Education of the Application for Licensure as Licensed Practical Nurse, the Application for Limited Permit, and paid the required fees.
  • Other requirements: See Special Qualifications below.
Licensed Practical Nurses – NC in the County service
  1. Must be able to reach, bend, etc. to provide patient care.
  2. Appropriate professional licensure must be maintained in good standing throughout employment.
  3. Applicants appointed under Option 2 must obtain licensure from the New York State Department of Education within 90 days of graduation from the nursing education program to be eligible for continuing employment.

NOTE: Livingston County residency is not a requirement for this position. However, Livingston County residents will be given reference in the hiring process.
