Job summary

BROWARD COMMUNITY & FAMILY HEALTH CENTERS, INC. is seeking a Staff Nurse (LPN) to join the care team and support providers in delivering primary health care.

The nurse will provide direct patient services, care coordination, and education, while performing designated procedures and working within PCMH quality improvement initiatives.

Qualifications

• Graduate from an accredited nursing school with LPN credential.
• Two years’ healthcare experience preferred.
• Florida nursing license required.

Job description

BROWARD COMMUNITY & FAMILY HEALTH CENTERS, INC.

POSITION TITLE: Staff Nurse (LPN)

CLASSIFICATION: Exempt

SUMMARY: The Staff Nurse works as part of the health center care team and assists the providers in the provision of primary health care. The Nurse provides direct patient services including care coordination and education. They also perform specific designated independent procedures and standing orders. They provide care coordination for patients of disease specific populations within the Patient Centered Medical Home. He or she is also responsible for the development, implementation and alignment of Broward Community & Family Health Centers (BCOM) goals and the existing quality improvement efforts and support the achievement of PCMH recognition by using data to better manage our patient population.

QUALIFICATIONS:

• Graduation from an accredited nursing school with a degree of Licensed Practical Nursing. Two (2) years’ experience in a health care or related setting preferred.
• License to practice nursing in the State of Florida.
• Excellent communication and interpersonal skills.
• Sensitivity in working with diverse cultural groups.
• Ability to present ideas and recommendations clearly and concisely in writing and orally with a proven ability to problem solve and solution oriented.
• Possess a strong ability to work independently and as a team member with fellow co-workers, supervisors and other staff.
• Demonstrates flexibility in work schedule and job assignments.
• General working knowledge of utilization management, performance improvement standards and procedures and NCQA Standards. Strong computer skills required.
