Qualifications

• Licensed Practical Nurse graduate from an accredited nursing program.
• Licensure required by state regulations.

Responsibilities

• Participate with interdisciplinary team to review, plan, coordinate and evaluate resident care.
• Document resident condition and nursing needs accurately and timely; report observations and reactions promptly.
• Develop, direct, and monitor nursing assistant assignments and adjust based on census and care level.
• Execute treatments as necessary and document reactions to medications and treatments.
• Generate, prepare, administer, and chart all medications.
• Initiate physician orders.
• Facilitate communication with families regarding medication changes.
• Ensure care plans are updated to reflect resident individuality and needs.
• Coordinate admissions, discharges, and transfers to deliver quality service.
