We are seeking a Licensed Practical Nurse (LPN) to support patient‑centered care delivery within Capital Health Plan's Nursing department. This role assists in providing direct patient care, health promotion, disease prevention, and patient education while working closely with providers and Registered Nurses to ensure care is coordinated, team‑based, and aligned with community needs.
The LPN identifies patient needs through in‑person, phone, and electronic interactions, documents care in clinical systems, supports care gap closure, and implements plans of care under the direction of a Registered Nurse or licensed provider. This role plays a key part in maintaining safe, compliant, and high‑quality patient care.
