Job summary

Casa de Paz Health Care Center in Sioux City, IA seeks an LPN to join a supportive interdisciplinary team and provide compassionate care to residents.

You will work under the guidance of registered nurses, administer medications, monitor health status, and assist with care planning while documenting care accurately. New graduates are welcome and training is provided.

Qualifications

• Iowa LPN license active.
• Graduate of an accredited practical nursing program.
• CPR certification or ability to obtain as required.
• Strong communication, organization, and teamwork skills.
• Long-term care experience preferred, but not required.
• New graduates welcome; hands-on training provided.

Responsibilities

• Provide nursing care and treatments per each resident's care plan.
• (Administer medications and treatments per regulations).
• Monitor residents' conditions and communicate concerns promptly to nursing leadership.
• Assist with resident assessments and care planning with RN and team.
• Maintain accurate and timely clinical documentation.
• Support residents' comfort, dignity, safety, and well-being.
• Communicate effectively with residents, families, physicians, and team members.
• Help create a positive environment where staff feel supported.
