Job summary

Qualifications

• Current, unrestricted LPN license in Georgia, and graduation from an accredited nursing program.

• Strong communication to educate patients and promote preventive health.
• Experience with EHR documentation and charting.
• Ability to work in a small primary care team with professionalism and respect.
• CPR/BLS certification and infection control adherence.
• Experience in primary care or outpatient settings preferred but not required.

Role Description

This part-time Licensed Practical Nurse role is an on-site position located in the Greater Savannah Area. The LPN will provide direct patient care, including obtaining vital signs, administering medications, performing injections, assisting with procedures, and documenting encounters in the medical record. Daily responsibilities include rooming patients, reviewing histories, supporting preventive care, coordinating follow-up appointments, and reinforcing patient education on treatment plans and wellness strategies. The LPN will collaborate with the primary care clinician to support chronic disease management, triage patient concerns, and promote evidence-based care. This role also involves maintaining a clean and safe clinical environment, managing supplies, and contributing to a supportive, patient-centered practice culture.

Qualifications

• Current, unrestricted Licensed Practical Nurse (LPN) license in the state of Georgia and graduation from an accredited nursing program.
• Clinical care skills, including vital signs assessment, medication administration, injections, basic wound care, and support for in-office procedures.
• Patient communication and education skills, with the ability to explain care plans, promote preventive health, and engage patients in self-management.
• Electronic health record (EHR) documentation, charting accuracy, and basic computer proficiency.
• Ability to work collaboratively in a small-team primary care setting, demonstrating reliability, professionalism, and respect for diverse backgrounds.
• Strong organizational skills for appointment support, follow-up coordination, and maintaining clinical supplies and equipment.
• CPR/BLS certification and adherence to infection control, safety, and privacy standards.
• Experience in primary care, direct primary care, or outpatient family medicine is preferred but not required.
