We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Responsibilities

  • Deliver comprehensive nursing care by performing physical examinations, assessing vital signs, and evaluating patient needs with precision and compassion.
  • Administer medications and injections accurately, including IV infusions and catheterizations, following established protocols and safety standards.
  • Assist with diagnostic evaluations such as specimen collection, processing, and medical imaging procedures to support accurate diagnoses.
  • Manage complex care plans for patients with chronic conditions like Alzheimer's disease, dementia, or those requiring hospice or palliative care.
  • Monitor patients’ responses to treatments, including pain management strategies and ventilator support for critical care cases.
  • Document all patient interactions thoroughly within electronic health records (EHR/EHR systems), ensuring compliance with HIPAA and confidentiality policies.
  • Collaborate effectively with interdisciplinary teams to coordinate discharge planning, case management, and client communication for optimal outcomes.

Qualifications

  • Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in good standing.
  • Experience working with diverse populations such as pediatric, geriatric patients, individuals with developmental disabilities, or those requiring specialized memory or behavioral health care.
  • knowledgeable in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for accurate medical documentation.
  • Knowledge of medical terminology, physiology, anatomy, ICD coding (ICD-9/10), and long-term care regulations.
  • Experience administering injections, performing venipuncture (phlebotomy), sterile techniques, and first aid procedures.
  • Ability to handle acute pain management cases and work efficiently in high-pressure environments like ICU or emergency medicine settings.
  • Excellent caregiver communication skills paired with a compassionate approach to patient assessment and client interaction.
  • Additional skills preferred include ICU experience, hospital medicine background (including Level I & II trauma centers), hospice or palliative care expertise, infectious disease management, tube feeding procedures, and familiarity with medical imaging techniques.
