Job summary

Paramount Community Living Center & Rehab in Newton, Kansas, is seeking a Licensed Practical Nurse (LPN) for all shifts. You will provide compassionate, direct resident care and support a collaborative care team.

Responsibilities include administering medications, wound care, monitoring vital signs, and coordinating with RNs, CNAs, physicians and therapists to ensure high-quality care. CPR/BLS certification is required.

Qualifications

• LPN/LVN licensure in Kansas.
• Graduation from an accredited LPN/LVN program.
• CPR/BLS certification required.
• Experience in a skilled nursing facility preferred.
• Excellent verbal and written communication.
• Physical ability to lift/move patients and stand for extended periods.

Responsibilities

• Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
• Monitor residents’ vital signs, document progress, and report significant changes to the Charge Nurse or Director of Nursing.
• Assist in developing and implementing individualized care plans.
• Maintain clear documentation and communicate with residents, families, and the healthcare team.
• Adhere to facility policies, infection control and safety protocols.
• Provide emotional support and education to residents and families.
• Collaborate with RNs, CNAs, physicians and therapists for coordinated care.

• Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.

Qualifications:

• Licensure: Valid LPN/LVN license in Kansas.
• Education: Graduation from an accredited LPN/LVN program.
• Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred.
• Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
• Communications: Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.
• Physical Requirements: Ability to lift and move patients, perform physical assessments, and stand for extended periods.
