Qualifications

Responsibilities

Summary

The Licensed Practical Nurse (LPN) provides skilled nursing care to individuals and groups under the direction of a Registered Nurse or licensed provider. The LPN assists in assessing, planning, delivering, and documenting patient care, supports patient/family education, and contributes to the overall quality and safety of the healthcare environment. Care is provided in accordance with ACH standards, regulatory requirements, and within the scope of practice for LPNs.

Responsibilities
  1. Provide age-appropriate nursing care under the supervision of a Registered Nurse (RN) or designated provider, applying basic knowledge of biological, physical, behavioral, social, and nursing sciences to meet the needs of individuals and groups.
  2. Collect and monitor patient data, including packaging and obtaining specimens and labs, contributing to assessments and promptly communicating abnormal findings or changes in condition to the appropriate provider.
  3. Participate in developing and implementing patient care and discharge plans under RN direction, deliver nursing care and treatments as ordered, and assist with personal care and activities of daily living to maintain hygiene and nutrition.
  4. Safely administer prescribed medications, injections, and treatments while monitoring for adverse reactions, and ensure all providers orders are followed for patient treatment.
  5. Provide education to patients, families, and caregivers regarding health, treatments, and self-care.
  6. Document all aspects of patient care in accordance with hospital policies and nursing care standards.
  7. Adhere to regulatory standards and ACH policies, maintain a safe and clean environment, and report safety concerns as appropriate.
  8. Work collaboratively as a member of the patient care team, engage in ongoing education and professional development, and perform additional duties to support efficient operations and quality patient care.
  9. Other duties as assigned.
Technical Expertise
  • Excellent patient care, verbal, and written communication skills.
  • Ability to work effectively with all levels within the organization.
  • Proficiency in MS Office Suite (Outlook, Excel, Word) or similar software.
  • Experience with electronic medical records (e.g., Epic) preferred.
  • Previous experience in healthcare, home care, or physician office settings preferred.
Education and Experience
  • Graduate from an accredited school of licensed practical nursing.
  • Current, valid LPN license in the state of Ohio.
  • Basic Life Support (BLS) certification from the American Heart Association. Additional certifications (e.g., PEARS, PALS, ACLS, TNCC, NRP) may be required based on assignment or department/unit.
  • Minimum one (1) year of relevant experience is preferred.
  • Years of supervisory experience: none.
