Job summary

REM Community Services, part of the Sevita family, is seeking a Licensed Practical Nurse to join our mission-driven team in West Virginia. This full-time position offers competitive pay and the chance to support individuals with developmental disabilities.

Responsibilities include monitoring health, administering medications, documenting care, and coordinating with staff and medical providers. An active WV LPN license and CPR/FA are required.

Qualifications

• State LPN or LVN license current in West Virginia.
• Associate degree or nursing diploma from an accredited program.
• CPR/FA certification and HIV/AIDS/BBP training as required by state rules.

Responsibilities

• Monitor health conditions of individuals served and ensure quality services.
• Administer medications and document; supervise staff who medicate.
• Provide care per physician orders and ISP plans.
• Conduct health assessments and respond to condition changes.
• Train staff on precautions, infection control, and med administration.
• Liaise with medical providers and advocates for individuals served.
• Educate individuals on self-care topics, including medication and preventive health.

Licensed Practical Nurse

Full Time Position Available

Pay: $20.40

Welcome Bonus: $3000

Responsibilities

• Monitor the health conditions of individuals served and ensure quality services are provided.
• Administer medications and provide documentation as well as oversee and monitor staff members who administer medications.
• Provide prescribed medical treatment by physician’s orders and personal care services.
• Conduct assessments, implement health care services to be included in each Individual Service Plan (ISP), and respond to changes of condition.
• Provide training to staff members including universal precautions, infection control, and medication administration.
• Serve as liaison and advocate with other medical and professional service providers.
• Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention and smoking cessation.

Qualifications

• Associate’s Degree in Nursing or related field, or a graduate of an accredited LPN program.
• One year of experience working with population served in a nursing capacity.
• Current state LPN or LVN License.
• Current driver's license, car registration, and auto insurance.
• Additional certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training.
• Ability to effectively communicate with staff, individuals served, and families.
