Job Summary:
Twin Fountains Home for Nursing and Rehabilitation is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated healthcare team in LaGrange, Georgia. As an LPN on our team, you will play a vital role in providing high-quality patient care to our residents, promoting their physical, emotional, and social well-being.
Responsibilities:
Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with interdisciplinary team members
Perform routine medical procedures, such as injections, nasal tracts, and Foley catheter care
Accurately document patient charts and maintain confidentiality
Promote a safe and comfortable environment for residents, visitors, and staff
Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
Requirements:
Current licensure as an LPN in the state of Georgia
High school diploma or equivalent required; Associate's degree in nursing or related field preferred
Minimum 0 years of experience in a long-term care setting; prior experience in a hospital or rehabilitation setting a plus
Basic Life Support (BLS) certification and CPR training
