Base pay range

$30.00/hr - $33.00/hr

In this role, you will play a vital part in facilitating patient discharge planning and ensuring effective communication between specialists, clinicians, and patients' families.

Specific Duties:

Ensure continuity of care in inpatient and outpatient settings, utilizing appropriate resources within established contracts and patients’ health plan benefits.
Perform care management functions on-site or telephonically as needed, working with the care team and PCP as the care team leader.
Meet with patients, families, and caregivers to discuss care and treatment plans as part of the Transitions of Care program.
Identify and follow up with high-risk patients in various care settings.
Consult with physicians and team members to implement care plans and coordinate treatments.

Qualifications and Experience:

Experience in acute clinical care, care management, utilization review, or discharge planning.
At least 1 year of experience in an acute clinical care setting.
HMO experience.
Current and valid Florida LPN license.
IV Certification.

Schedule:

Shift: 8:00 AM - 5:00 PM
Days: Monday - Friday

Dress Code:

Scrub top and bottom with closed-toe shoes.
Colors: Navy blue or black.
