Job summary

RHA Health Services, LLC is seeking a Licensed Practical Nurse in Humboldt, TN to provide holistic nursing care for individuals with IDD and mental health/substance use challenges. You will assist the RN, train unlicensed staff, and help coordinate medications and health services.

The role requires an LPN license, CPR certification, and the ability to work independently in a fast-paced environment while effectively communicating with the care team and families.

Qualifications

• Graduate of an accredited licensed nurse program.
• Current nursing license with no restrictions to practice.
• Current CPR certification or eligibility to obtain certification.
• Strong communication with Interdisciplinary Team.
• Ability to work independently and prioritize tasks.

Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities.
• Assist the Registered Nurse in delivering quality nursing services to persons supported.
• Provide health related training to unlicensed personnel.
• Administer medications and treatments or assist with medication administration.
• Assist with scheduling preventative and routine medical appointments.
• Conduct medication close assessments and document findings.
• Collect health data and report abnormal findings to the physician.
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team.
• Advocate for the people supported and develop community supports for health.
• Support care team to promote optimal health outcomes for individuals supported.
