Job summary

Trustwell Living at Oakley Place is seeking a Licensed Practical Nurse (LPN) to provide direct resident care and supervise the care team per each resident’s plan. You will monitor changes in condition, document concerns, and coordinate with the Health Services Director to keep plans up to date and compliant with regulations.

A valid LPN license, at least one year in assisted living or long-term care, experience with medications, and a current driver’s license are required.

Qualifications

• A passion for seniors and enhancing their quality of life.
• Valid LPN license; compact license or ability to obtain one encouraged; unencumbered LPN license required.
• Minimum of one year of experience as a LPN in assisted living or long-term care.
• Experience administering medications and treatment plans per orders and policy.
• Knowledge of state regulations for resident care and survey readiness.
• Experience as a mandatory reporter and ability to intervene/report investigations.
• Current driver’s license.

As the Licensed Practical Nurse (LPN), you will be responsible for providing direct resident care and supervision of residents’ needs based on their individualized service/care plan. You will monitor residents for changes in condition and signs/symptoms of illness, in accordance with federal and state laws and regulations, as well as the policies of the Community. In partnership with the Health Services Director, you will communicate and document changes or concerns regarding residents and assist in supervising the care team.

The successful candidate for this position will have:

• A passion for seniors and enhancing their quality of life
• A valid Licensed Practical Nurse (LPN) license; a Compact license or the ability to obtain one is encouraged. An unencumbered LPN license is required.
• Minimum of at least one year of successful experience as a LPN in assisted living or long-term care
• Experience administering medication and treatment plans per doctor's orders and community policies
• Knowledge of state regulations pertaining to resident care and understanding the importance of being state survey ready, every day.
• Have experience working as a mandatory reporter and can immediately intervene, report, and investigate all suspected or alleged verbal, physical, sexual abuse, or theft from a resident.
• Have a current driver’s license.
