Job summary

Community Health Services is hiring a Licensed Practical Nurse (LPN) for our Fremont, OH location. The position is full-time with day shifts and a schedule that avoids weekends, offering strong benefits and paid holidays.

You’ll perform patient assessments, point-of-care testing, and support lab and diagnostic follow-up under the Nursing Supervisor. Ideal candidates hold Ohio LPN licensure, current BLS, and experience with EHR documentation.

Qualifications

• Graduate of a current accredited Nursing Program.
• Current Ohio LPN license.
• Current Basic Life Support Certification (BLS).
• Minimum of one year as an LPN preferred.
• Knowledge of documenting with EHR systems.
• Strong communication skills and ability to multi-task.

Responsibilities

• Work directly with patients Monday through Friday—no weekends.
• Perform adult and pediatric nursing assessments.
• Communicate clearly with Providers, Patients, and CHS Staff.
• Follow-up on Lab and Diagnostic testing.
• Administer injections as needed.
• Follow all safety, HIPAA, and regulatory guidelines.

Job Details Job Location: Main - Fremont, OH 43420 Position Type: Full Time Salary Range: $25.50 Job Shift: Day Come to work with us at Community Health Services! We offer full-time benefits, 10 paid holidays, no weekend hours and so much more!

We are looking to hire a skilled Licensed Practical Nurse (LPN) at our Fremont, Ohio office, with the flexibility to support other locations as needed. You would be working alongside a dynamic team of healthcare professionals allowing you to utilize your skills and grow in your profession. Under the direction of our Nursing Supervisor, you would be responsible for the assessment of patients, point of care testing, lab and diagnostic follow-up, and other nursing functions to support quality patient care.

Hours for this position are:

Mondays 7am-7pm, Tuesdays through Thursdays 8am-5pm, Fridays 8am-1pm
