Elizabeth Park Rehabilitation and Nursing LLC in Elizabeth, NJ is seeking a compassionate Licensed Practical Nurse (LPN) for a 7:00 AM to 3:30 PM shift. The role emphasizes direct patient care, medication administration, and collaboration with a multidisciplinary team to deliver high-quality care in a long-term and rehabilitative setting.

Qualifications

• Must hold current LPN license in New Jersey.
• High school diploma or equivalent required; ADN/BSN degree preferred.
• 1+ year of LPN experience in long-term care or rehabilitation setting preferred.

Responsibilities

• Provide direct patient care including medications, wound care, and treatments as prescribed.
• Develop and implement individualized care plans with patients and families.
• Monitor patient progress and report changes to the care team.
• Collaborate with RNs, PTs, OTs, and social workers for comprehensive care.
• Educate patients and families to support understanding and empowerment.
• Maintain accurate records in compliance with regulatory standards.

Responsibilities

• Provide direct care to patients and residents, including administering medications, wound care, and other treatments as prescribed by physicians




• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams




• Monitor and report patient progress, identifying areas for improvement and making recommendations for modifications to care plans




• Collaborate with other healthcare professionals, including RNs, PTs, OTs, and social workers, to ensure comprehensive care




• Participate in patient and family education, promoting effective communication and empowerment




• Maintain accurate and detailed records, adhering to regulatory and facility standards

Requirements

• Current LPN licensure in the state of New Jersey




• High school diploma or equivalent required; ADN or BSN degree preferred




• 1+ year of experience as an LPN in a long‑term care or rehabilitation setting; experience in rehabilitation and nursing highly valued




• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams




• Ability to lift, transfer, and reposition patients, as well as perform other physical demands of the role
