You will conduct resident assessments, support care planning, administer medications, and maintain clear clinical documentation while collaborating with families and physicians.

Qualifications

• Active LPN license in good standing.
• Experience in assisted living, memory care, or senior living.

Responsibilities

Position Summary

The LPN / Licensed Practical Nurse plays a vital role in supporting the health, wellness, and quality of life of residents within our Assisted Living and/or Memory Care communities.

This position is responsible for resident assessments, care planning support, medication and treatment management, clinical documentation, and ongoing communication with residents, families, physicians, and caregiving teams.
