The Licensed Practical Nurse (LPN) provides high-quality nursing care and supports the daily clinical needs of residents in accordance with physician orders, individualized care plans, company policies, and applicable regulations.
Provide compassionate, resident-centered nursing care.
Administer medications and treatments accurately and timely.
Monitor residents and report changes in condition.
Complete accurate and timely clinical documentation.
Communicate with physicians, families, pharmacy, and other members of the care team.
Assist with admissions, transfers, discharges, and ongoing care planning.
Provide guidance and support to CNAs, PCAs, Care Partners, and other direct-care team members.
Respond appropriately to emergencies and changes in resident condition.
Follow infection prevention, medication management, safety, and quality standards.
Protect resident dignity, privacy, independence, and confidentiality.
Promote a positive and collaborative environment for residents and coworkers.
Current Licensed Practical Nurse (LPN/LVN) license in the applicable state, in good standing.
Graduate of an approved practical nursing program.
CPR/BLS certification as required.
Strong communication, organization, and clinical skills.
Compassionate and professional approach to resident care.
Senior living, assisted living, memory care, skilled nursing, rehabilitation, or long-term care experience is preferred but not required.
