Position Summary

Title: Licensed Practical Nurse
Location: Peekskill
Job Type: Full Time
Pay Range: $29.00 – $33.02 per hour
Sign-On Bonus: $2,700 (eligible external applicants only subject to terms and conditions)

Sun River Health is a federally qualified, non‑profit health center serving underserved communities across multiple counties. We seek a dedicated LPN to support patient care at our Peekskill site, working in partnership with medical staff and the broader health team.

Essential Duties and Responsibilities

• Conduct patient interviews and maintain medical records, including nursing observation, health history, vital signs, vision/hearing tests, medication administration and treatment, test reactions, laboratory results, and patient identifiers. Ensure documentation and use of clinical decision support systems.
• Perform in‑house laboratory tests and collect specimens for outside labs; conduct quality control per organizational protocols.
• Complete and update all assigned tasks and logs in a timely manner.
• Initiate follow‑ups as directed by providers, such as encounter documentation, lab orders, and closure of non‑billables.
• Provide health teaching as indicated by physicians, diagnoses, or patient inquiries, and direct patients to a Registered Nurse when necessary.
• Collect patient data under the supervision of a Registered Nurse or provider. Do not triage, interpret clinical data, or act independently on such data.

Essential Job Qualifications

• Minimum Education Requirement: Vocational/Technical School or Diploma Program – Nursing.
• Minimum Licensure:
  • Valid New York State LPN license.
  • CPR certification.
  • Infection Control certification.
