Job summary

Liberty Care & Rehabilitation Center in Kentucky is seeking a compassionate Licensed Practical Nurse to provide direct bedside care in our skilled nursing and rehab community. The LPN will administer medications, monitor vital signs, document accurately in electronic records, and coordinate with RNs, physicians, and therapists to support individualized care plans.

You will educate residents and families, support activities of daily living, and respond promptly to changes in condition.

Qualifications

• Licensed practical nurse with an active LPN license.
• Experience in skilled nursing or rehab settings preferred.
• Ability to document accurately in electronic records.

Responsibilities

• Administer medications and treatments to residents.
