Job summary

Liberty Care & Rehabilitation Center is seeking a compassionate Licensed Practical Nurse to provide direct bedside care to residents in our skilled nursing and rehab community. The LPN will administer medications and treatments, monitor vital signs and resident conditions, document accurately in electronic records, and coordinate with RNs, physicians, and therapists to support individualized care plans.

Qualifications

• Must hold an active LPN license in good standing.
• Provide direct bedside nursing care in skilled nursing/rehab.
• Administer medications and treatments as prescribed.
• Monitor vital signs and resident conditions continuously.
• Document accurately in electronic health records.
• Coordinate with RNs, physicians and therapists for care plans.
• Educate residents and families about care plans.
• Assist with ADLs and respond promptly to changes in condition.

Responsibilities

• Provide direct bedside nursing care to residents in skilled nursing and rehab.
