BrightStar Care is seeking a Licensed Practical Nurse (LPN) to deliver compassionate home health care directly to clients in their homes around Boston. You will be part of a supportive, high-performing team dedicated to improving lives.

• Active and valid LPN license in Massachusetts.
• Minimum 1 year of experience in home health or related care.
• Current BLS certification and reliable transportation to patient homes.
• Proficiency in electronic health records (EHR) and strong communication skills.

Responsibilities

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document patient vital signs and overall health status.
• Assist with daily living activities such as bathing, grooming, and meal preparation.
• Educate patients and families on managing health conditions and care plans.
• Coordinate with RNs, therapists, physicians, and other healthcare professionals.
• Maintain accurate and timely documentation using EHR systems.
• Provide emotional support and companionship.
• Follow all safety and health regulations in home care settings.
