Job summary

Berlin Rehabilitation and Healthcare Center is seeking an LPN to join our sub-acute and long-term care team. This role involves delivering compassionate nursing care, following individualized plans, and collaborating with an interdisciplinary team.

Qualifications

• Active LPN license in New Jersey.
• BLS/CPR certification required.
• Experience in Long-Term Care or sub-acute care.
• Familiarity with PointClickCare (PCC).
• Ability to manage multiple patients in a fast-paced setting.
• Strong communication and clinical judgment.

Responsibilities

• Provide patient care in accordance with the plan of care and physician orders.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments.
• Monitor residents’ conditions and report changes promptly to the supervising nurse or physician.
• Maintain accurate documentation per physician orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.

Same Day Pay available!

Full-Time: 7PM-7AM (Temp position August-November)

Part-Time: 7AM-7PM, 7PM-7AM, 7AM-3PM, 3PM-11PM

PER-DIEMS: 7 AM - 3 PM, 3 PM - 11 PM, 7 AM - 7 PM, 7 PM - 7 AM

$30.00 - $35.00 per hour (all inclusive)

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.

• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.

• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.

• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of New Jersey.

• Valid BLS Certification (CPR) required.

• Prior experience in Long-Term Care, Sub acute setting.

• Prior experience with PointClickCare (PCC).

• Ability to care for and manage multiple patients in a fast-paced healthcare environment.

• Strong clinical judgment, communication skills, and attention to detail.
