Job summary

Addus HomeCare in Tennessee seeks a licensed practical nurse to provide direct patient care in home health settings. You will implement and document the plan of care, assess patient needs, and educate families to support self-care.

The role requires an active TN LPN license, CPR certification, and reliable transportation. You’ll collaborate with RNs and other disciplines to coordinate home health services while maintaining thorough, timely documentation.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; community/home health or medical/surgical preferred.
• Current TN license as LPN/LVN.
• Possess and maintain valid CPR certification.
• Reliable transportation, current driver''s license and automobile insurance.

Responsibilities

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, reporting needs for other disciplines to the RN/Clinical Supervisor.
• Uses and/or prepares equipment and supplies adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the RN.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.

What You’ll Do:
