Job summary

The Maples Clinic in Hammond, LA is seeking a Licensed Practical Nurse for PRN shifts to support our multidisciplinary team under RN supervision. You will provide basic patient care, take vital signs, administer medications, assist with exams, and thoroughly document patient care.

• Must hold an active LPN license in Louisiana.
• Graduate of an accredited school of nursing.
• Minimum 1 year LPN experience in behavioral health preferred.
• BLS certification.
• NVCI certification.

Responsibilities

• Participates in planning, coordination, and documentation of total patient care with the treatment team.
• Reports and records client condition and care, noting changes.
• Monitor skin assessments and document changes.
• Maintain accurate clinical notes and documentation.
• Ensure timely documentation for nursing services provided.
• Participates in trainings and meetings as needed.
• Responds to directives and adapt to a changing healthcare environment.
• Follow and document MD orders during shifts.
• Manage medication administration and storage according to policies.

What does the Nurse do? (including but not limited to)

As a Nurse, you are responsible for providing basic patient care under the supervision of a registered nurse, including taking vital signs, administering medications, preparing patients for exams, collecting specimens, assisting with procedures, educating patients on self-care, and documenting patient information thoroughly; all while maintaining clear communication with the healthcare team and patients.

Responsibilities – Essential duties and responsibilities (including but not limited to)

• Participates in the planning, coordinating, and documenting of total patient care in conjunction with the multidisciplinary treatment team
• Accurately reports and records the client’s condition and care, including signs and symptoms which may be indicative of change
• Accurately monitor skin assessments with proper documentation and reporting of changes
• Completes, maintains, and documents accurate and relevant clinical notes
• Ensures timely and accurate clinical documentation for all nursing clinical services provided
• Participation in education, in-services, and other training and meetings as needed
• Facilitates appropriate nursing action(s) in response to administrative directives and a changing healthcare environment
• Ensures all MD orders are followed and documented accordingly during the assigned shift
• Manages medication administration, monitoring, and proper storage of medication and complies with all medication management policies and procedures

Qualifications

• Graduate of an accredited school of nursing
• Must have active Licensed Practical Nurse in the state of Louisiana.
• Minimum of 1 year experience as an LPN experience in behavioral health care preferred
• Certification in Basic Life Support
• Certification in non-violent crisis intervention
