Job summary

Apploi is seeking a licensed Physical Therapist in Florida to plan, deliver, and supervise physical therapy services within a long‑term care facility. You will evaluate outcomes, guide discharge planning, and ensure compliant documentation and billing for residents receiving PT services.

The role requires knowledge of state/federal regulations, the ability to coordinate KPI reporting, and collaboration with an interdisciplinary team to optimize resident functional status and care quality.

Qualifications

• Must hold a valid, unrestricted license in the therapy discipline in the state of employment.
• Must meet educational requirements for licensure.
• Valid driver’s license in the state of employment.
• Knowledgeable of skilled nursing home regulations, procedures and long‑term care guidelines.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct delivery of physical therapy services.
• Coordinate facility KPI reporting for physical therapy indicators.
• Evaluate KPI outcomes and report to Rehabilitation Director for action if needed.
• Lead processes for timely screening, evaluation, treatment, discharge and documentation for residents.
• Ensure proper documentation, billing and coding for residents receiving PT services.
• Provide resident information for assessments (functional abilities, goals, ROM, balance, walking) during observation periods.
• Participate in interdisciplinary meetings to discuss therapy progression and discharge planning.
• Ensure PT equipment is stored, used, and sanitized safely and correctly.
