Qualifications

Responsibilities

• Consult with patients to assess physical condition and review medical history.
• Diagnose movement dysfunction by observing patients and evaluating movement.
• Develop individualized treatment plans tailored to each patient.
• Coach patients and teach therapeutic massage techniques to ease pain and promote healing.
• Maintain accurate patient records and track goals and progress.
• Utilize standardized assessments and evidence-based practice to guide interventions.
• Assess patient progress and adjust treatments in the clinical record per requirements.
• Evaluate treatment outcomes and modify services based on patient responses and referrals.
• Provide consultation to patients, families, and caregivers about physical disorders.
• Communicate progress and plans with supervisor and health team members.
• Provide comprehensive discharge summaries per regulatory requirements.
• Document all regulatory and clinical information in the electronic system and maintain records.
• Participate in required trainings, in-services, and conferences.
• Able to supervise and support PT assistants and rehab aides.
• Adhere to company policies and code of conduct.
• Maintain active State Licensure.
