Qualifications

• Active license as Physical Therapist in the state of practice.
• Graduation from a CAPTE-accredited PT program.

Responsibilities

• Perform evaluations, assessments, and treatments per the PT care plan.
• Complete documentation accurately and timely.
• Follow policies and procedures within scope of practice.
• Prioritize patient care with initiative and sound judgment.
• Complete orientations and trainings as required.
• Perform duties per supervisor's direction.
