EmpowerMe Wellness is seeking qualified therapists to deliver therapy and wellness services to older adults within our EmpowerMe 360 program. You will assess residents, create personalized care plans, and collaborate with a multidisciplinary team to support independence and safety.

The role emphasizes resident-centered care, family caregiver engagement, and participation in program development and quality improvement, with opportunities for leadership and professional growth.

Qualifications

• Licensed therapy professional for seniors (PT/OT/SLP) with passion for empowering seniors.
• Ability to conduct assessments and develop personalized care plans.
• Strong collaboration with care teams, caregivers, and residents.
• Commitment to ethical standards and quality improvement.

Responsibilities

• Provide therapy and wellness services for older adults.
• Develop assessments and personalized care plans with evidence-based interventions.
• Support care coordination, caregiver communication, and telehealth when appropriate.
• Maintain professional standards aligned with company initiatives.
• Contribute to program development and quality improvement initiatives.
• Help residents maximize independence, confidence, and satisfaction.
