Licensed Physical Therapist Assistant (PTA) - Care Coordination Program

Physical Therapy + Care Coordination: The Best of Both Worlds!

We’re seeking Physical Therapy Assistants to support our innovative care coordination program. This role allows you to practice at the top of your license, expand into care coordination duties, and offers flexible scheduling, excellent benefits, and a meaningful impact on seniors’ lives.

Responsibilities:

The Opportunity

Join our team to work with residents in senior living communities, including independent, assisted living, and memory care settings. Therapy services are covered by private insurance and/or Medicare Part B.

Your responsibilities include:

  • Providing physical therapy and wellness services under supervision of a PT
  • Supporting care coordination through wellness check visits, communication with providers and caregivers, and assisting with telehealth visits
  • Maintaining professional and ethical standards
  • Contributing to program development and quality improvement
  • Enhancing each patient’s functional well-being and satisfaction

Your Schedule

Our clinics operate Monday to Friday, 8 a.m. to 5 p.m., but flexible scheduling allows you to tailor your hours to your lifestyle.
