Licensed Physical Therapist Assistant PTA - Care Coordination - Part-Time

We are seeking a licensed Physical Therapist Assistant (PTA) to join our innovative care coordination program, supporting senior residents in various care settings including independent living, assisted living, and memory care. This role involves providing therapy services, supporting care coordination efforts, and contributing to program development, all while enjoying flexible scheduling and excellent benefits.

Responsibilities include:

  • Providing physical therapy and wellness services under supervision
  • Conducting wellness check visits and communicating with healthcare providers and caregivers
  • Maintaining professional standards and contributing to quality improvement
  • Assisting in optimizing patients' functional well-being

Your schedule: Our clinic hours are typically 8 a.m. to 5 p.m., Monday through Friday, with flexible scheduling options to suit your lifestyle.
