HealthPro Heritage is seeking a Full-time Licensed Physical Therapist Assistant for our retirement community in South Setauket, NY. Join a supportive, patient-focused team delivering quality care in a rehab-friendly environment.

As an LPTA you will assist with treatment plans, provide direct patient care, document progress, and educate patients and families on exercises and self-care. You’ll collaborate with therapists and staff to drive meaningful outcomes and professional growth.

Qualifications

• Associate degree in Physical Therapist Assisting from an accredited program.
• Current state licensure as a Physical Therapist Assistant or licensure eligible.
• Strong assessment, diagnostic, and therapeutic skills with excellent communication.

Responsibilities

• Assist in implementing treatment plans created by Physical Therapists.
• Provide direct patient care including exercises, manual therapy, and modalities.
• Document patient progress and report outcomes per facility policies.
• Collaborate with therapists and care teams to ensure coordinated care.
