We are seeking a Per Diem LPN/LVN to support our home health patients. The Licensed Practical Nurse/Licensed Vocational Nurse will deliver skilled nursing care and supportive services under the supervision of a Registered Nurse, adhering to company standards and legal regulations.

As a Home Health LPN, your responsibilities include:

• Reporting directly to the Administrator or Executive Director.
• Providing direct patient care, including treatment of illnesses, rehabilitation, and preventive care.
• Evaluating patient and family needs and assisting in developing and implementing care plans.
• Reporting changes in patient conditions to the RN, interdisciplinary team, and physicians, with proper documentation.
• Attending staff meetings.

Home Health LPN Requirements:

• Comfortable providing care in the patient's home environment.
• Valid LPN/LVN license in the state of employment.
• At least 1 year of patient care experience in hospice, home health, ICU, geriatrics, or oncology is preferred.
• Additional relevant experience such as hospital staff LPN/LVN, Med Surg, ER, critical care, wound care, telemetry, or nursing home experience is advantageous.
• Current driver’s license and automobile insurance.
• Current CPR certification.
