Job summary

4C Health in Indiana is seeking a licensed outpatient therapist to provide high-quality mental health, addictions, and behavioral health services. The role operates under supervision for licensure and includes a 4-day, 32-hour work week with dedicated supervision and administrative time.

This position provides assessments and therapeutic services to individuals, couples, families, and groups. This position works to provide mental health, addictions and behavioral health clinical treatment/diagnosis within scope of practice as defined by degree and license. This position is responsible for participating in treatment planning with the center psychiatrist and client, reporting client progress to appropriate referral source, and maintain adequate and timely documentation. This position operates under minimal supervision and must exercise tact, judgment and confidentiality in contact with the public, clients and families, as well as fellow associates of the Center.
