Job summary

Independence Health System in Butler Township, OH seeks a licensed Occupational Therapist to join our Rehabilitation team. You will evaluate patients, establish plans of care, set measurable goals, and collaborate with physicians and staff to optimize outcomes.

Responsibilities include documenting care in EMR, coordinating discharge planning, educating patients and families, and participating in program development to enhance care across the system. Evening/weekend shifts may apply.

Qualifications

• Graduate of an accredited OT program.
• Strong leadership, organizational, and communication skills.
• Knowledge of current billing requirements and payor guidelines.
• Ability to work independently and in a team setting.

Responsibilities

• Evaluate patients and establish plans of care with measurable outcomes.
• Deliver age-appropriate therapy and modify treatment as needed.
• Document care in EMR including evaluations, treatment and progress.
• Assist with discharge planning and patient/family education.
• Supervise COTA, students, and support personnel when appropriate.
• Participate in program development and process improvement.
