Job summary

EmpowerMe Wellness is seeking therapists to provide therapy and wellness services to older adults within EmpowerMe 360. You will conduct assessments, develop personalized care plans, and deliver evidence-based interventions as part of a collaborative team.

You will support care coordination, communicate with caregivers, and offer telehealth options while maintaining professional standards and contributing to program improvements. Your role helps residents maximize independence and satisfaction.

Qualifications

• Current professional license in your therapy discipline (PT, PTA, OT, COTA, or SLP).
• Passion for empowering seniors to live their best lives.
