Job summary

Azria Health Park Place is dedicated to delivering quality occupational therapy services to residents in Des Moines. The OT will evaluate patients, develop and implement care plans, and collaborate with physicians to adjust treatments while educating patients and families for functional improvement.

The role requires Iowa OT licensure, strong adherence to safety and confidentiality, and participation in ongoing professional development.

Qualifications

• Must be licensed as an Occupational Therapist in Iowa.
• Regular and consistent attendance is essential.
• Able to perform physical tasks including lifting up to 50 lbs.
• Description may change and is not a contract of employment terms.

Responsibilities

• Assess and direct patient care and treatment plans.
• Evaluate patients using appropriate Occupational Therapy techniques.
• Develop and implement patient plans of care and goals.
• Educate and motivate patients and families to learn and improve functional activities.
• Document progress, discharge summaries, and home programs.
• Coordinate with physicians and staff to modify and coordinate treatment plans.
• Maintain confidentiality of patient information and ensure safety and compliance with policies.
• Assist with cross-functional duties to ensure seamless patient care.
