Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities.

In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Current OT license in Michigan or eligibility to practice in Michigan.
• Ability to assess residents and create individualized treatment plans.
• Strong documentation and communication with the care team.

Responsibilities

• Evaluate residents in skilled nursing and post-acute facilities.
• Develop individualized treatment plans focused on ADLs, mobility, cognition and safety.
