Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role entails evaluating residents, creating individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active OT license in the state of Illinois
• Experience delivering patient-centered care in skilled nursing or post-acute settings preferred
• Proficient in EMR documentation and interdisciplinary collaboration
