Job summary

Personal Touch Home Health Care Services Inc. is seeking a Licensed Occupational Therapist to provide compassionate, evidence-based rehabilitation to primarily senior patients in skilled nursing and post-acute care settings.

You will conduct thorough evaluations, develop individualized treatment plans, and deliver goal-oriented interventions to improve safety, independence, and quality of life. Collaborating with an interdisciplinary team, you will document progress in EMRs and educate patients
