Under the general supervision of the Director of Acute Care, is responsible for performing various duties and tasks as requested under the LNA scope of practice as determined by the New Hampshire Board of Nursing.

Clinical Competency

  • Under the direct supervision of a registered nurse assists with patient care activities, activities of daily living, hygiene, feeding, and patient safety initiatives. Promptly reports any observed changes in patient's condition to nurse or physician
  • Performs and documents vital signs, height, weight, intake, and output and reports any changes out of range of normal, performs 12 lead EKGs
  • Obtains laboratory specimens as appropriate, i.e., finger stick glucose testing, urine specimens and transports to lab as necessary
  • Within scope of practice, assists nurse/physician with clinical procedures and treatments
  • Carries out roles assigned as part of the healthcare team during emergencies or resuscitation efforts (CPR, runner, order entry, documentation, etc.)
  • Answers phone professionally with courtesy to caller, relays messages accurately and timely. Contacts other departments and agencies via phone, as requested
  • Cross trained to perform medial unit clerical duties; computer charting, inputting orders, filing of patient charts
  • Performs safe transportation of patients within the facility utilizing safe lifting and transport
  • Performs post-mortem care and aids in the transportation of the body to the morgue
  • Demonstrates safe, competent use of patient care equipment, beds, stretchers, lifts, safety and alarm devices, walkers, and other equipment approved for patient care

  • Displays ability to work independently and within a group
  • Adheres to standards and applicable codes of conduct that govern behavior among peer and colleagues to create an environment promoting cooperation, respect and trust
  • Maintains professionalism by adhering to the Huggins Hospital dress code
  • Continues professional growth by maintaining education hours for license renewals
  • Maintains active LNA license and BCLS. Adheres to the LNA standards of care as defined by the NH Board of Nursing

Patient Advocacy

  • Sets the stage for a positive patient interaction by always introducing self and clinical role
  • Facilitates patient and family centered care via clear communication of plan of care and educating at a level the patient, family and/or caregiver can comprehend
  • Promotes a safe patient care environment safe from hazards. Performs safety watches, 1:1 patient observation as assigned, completes documentation for 1:1 observations. Maintains safe use of safety adjuncts (bed alarms, hover mats, gait belts, walkers, etc.)
  • Maintains cleanliness of patient rooms, workstation and common areas; assist with restocking and ordering of medical supplies and medical equipment as needed
  • Recognizes and utilizes appropriate resources for non-English speaking patients, healthcare illiterate, hearing and sight impaired individuals
  • Delivers age specific, individualized patient care Take proactive steps to recognize and report suspected abuse/neglect including child, elder, sexual, spousal, and reports to supervisor

Quality

  • Adheres to Infection Prevention standards, consistent in hand hygiene practice before and after each patient interaction. Adheres to infection prevention precautions. Holds team members to IP and hand hygiene standards and room/patient IP precaution measures
  • Promptly responds to patient calls via Canfield system, assist patients and locates and relays messages to nursing personnel as patient condition/situation requires
  • Participates in quality review, performance improvement and patient satisfaction initiatives within the department (such as hourly rounding). Participated in unit continuous CMS survey readiness activities

Organizational Expectations

  • Attendance: Attendance is an essential job function; employee comes to work daily, on time, and completes scheduled shift
  • HIPAA: facilitates to maintain patient confidentiality
  • Internal communication: contributes to positive team performance and a culture of service excellence through effective communication, active listening, and respect
  • Interpersonal relationships: maintains appropriate interpersonal relationships with staff, patients, and visitors at all times. Works as a team member with other departments.
  • Safety: Practices workplace safety daily

Employee Engagement

  • Actively participates in all hospital Service Excellence initiatives and trainings
  • Attends and participates in a minimum of 50% department staff meetings and signs off on review of 100% department staff meeting minutes
  • Completes all required annual education on or before the due date

Knowledge Skills and Abilities: Individuals must possess these knowledge, skills, and abilities or be able to explain and demonstrate that they can perform the primary functions of the job, with or without a reasonable accommodation, using some combination of skills and abilities and possess the necessary physical requirements, with or without a reasonable accommodation, to safely perform the essential functions of the job.

  • Minimal physical effort required with occasional light lifting to a maximum of 25 pounds; stooping; and pick, pinch, type, or otherwise work primarily with fingers
  • Ability to effectively manage considerable mental stress
  • Ability to express or exchange ideas by means of the spoken word
  • Ability to receive detailed information through oral communication
  • Ability to communicate effectively with co-workers and convey information in a clear, concise and accurate manner
  • Familiarity with computer and other business machines

Qualifications

Education: Graduate from a nursing assistant program

Experience: 1 year experience as an LNA preferred, experience in a hospital acute care setting preferred.

Computer Literate, Experience With Electronic Health Records Preferred

License and Certification: New Hampshire LNA license, BLS Certification
